Transportation:

We provide transportation. We offer families of 2 or more children transportation during the months of September to June for free.

Registration Forms:

Forms must be filled out completely. When information changes, you must notify the center to change your records. The emergency contact information is very important and we will not release a child to anyone not listed on this form.

Medical Forms:

All children are required to have all of their immunization shots up to date. You have 30 days to have your doctor or clinic fill out this form or your child may not return to the center.

Medications:

All meds are to be sent to the center daily and must be in the original bottle with written Drs. directions. We have a daily log form that you must sign.

Meals:

All meals are provided.

Items the children need:

Book bag, complete change of clothes, wash cloth, sleeping bag or blanket (which must be washed each week)

Lost & Found:

We assume no responsibility for lost or broken items. Let us know immediately if there are any mix ups or losses. Please do not allow children to wear expensive jewelry to class
